ONE PERSON per appointment slot. Each child must have their own appointment slot.



This office ONLY accepts passport applications for: 

  • New passport applications (minors and adults). All applications for minors are considered new as they do not renew.
  • Name changes (with valid original documentation).
  • Lost or stolen passports.

For ADULT RENEWALS (Passport was issued at age 16 or older):  

  • Please visit U.S. Passports (state.gov) and complete the renewal process there.  
  • Adult Renewals must be done by MAIL ONLY and will not need an appointment.
  • Please do not book an appointment for renewals as this office does not process adult passport renewals.
  • Expired passports may be renewed up to 5 years after expiration.

WHAT TO BRING:
Applicants must have all items listed below or passport cannot be processed. No exceptions.
  1. DS-11 Form. (Application for a New US Passport) filled out in black ink. Please complete but do not sign this form until instructed by the passport agent. The social security number is required on all applications and all applicants must be present. Please print out all forms on one sided 8 1/2 x 11 pages only. Application will not be accepted if print if not legible or printed on both sides. Applications are also accessible and may be printed from www.travel.state.gov
  2. Proof of Citizenship. The following are acceptable documents:
    1. An original birth certificate (must have the full name of both parents) and/or
    2. Naturalization certificate or
    3. An expired passport
    4. Only original documents are accepted AND must be submitted at the time of application.  Original documents will be returned after the new passport has been issued. Photocopies are not valid as proof of citizenship. Hospital birth certificates and Abstracts of Birth are also not accepted.  Electronic U.S. birth certificates cannot be used when applying for a passport. Passport Services only accepts certified U.S. birth certificates issued directly by a state or local vital records office. These birth certificates are issued on special paper with security features to mitigate the risk of fraud.
  3. Proof of Identity (Submit one):
    • Valid or expired, undamaged U.S. passport book or passport card  
    • In-state, fully-valid driver's license or enhanced driver's license with photo
    • Certificate of Naturalization  
    • Certificate of Citizenship  
    • Government employee ID (city, county, state, or federal) 
    • U.S. military or military dependent ID 
    • Current (valid) foreign passport 
    • Matricula Consular (Mexican Consular ID) - commonly used by a parent of a U.S. citizen child applicant 
    • U.S. Permanent Resident Card (Green Card) - commonly used by a parent of a U.S. citizen child applicant 
    • Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards) 
    • Enhanced Tribal Cards and Native American tribal photo IDs 
    • Other documents: 1) In-state, fully-valid learner's permit with photo, 2) In-state, fully-valid non-driver ID with photo, or 3) temporary driver's license with photo. Note: you may be asked to present an additional ID when presenting one of these three documents. 
    • Notice on Digital Identification Documents: Some states now issue digital ID documents (also known as mobile driver's licenses or mobile IDs). We cannot accept these digital IDs when you apply for your U.S. passport. You must continue to submit your physical, photo ID and a photocopy of the ID. 
  4.  One New Passport Photo. (Please click here to see the Photo Requirements for passports.) Photos may not be more than 6 months old.  Please note, WE DO NOT take photos at this facility. 
  5. Payments. 2 payments MUST be made at the time of appointment:
    1. Check or money order payable to the Department of State for the Application Fee (see below for complete list of fees) AND
    2. Credit card (Visa/MC) payment for $35 City Processing Fee.
    3. NOTE: Each application needs a separate check.
Special Requirements for Minors Age 16-17
Minors 16-17 need to have their own ID (California or School ID is sufficient), and only need to be accompanied by one parent, or have proof of parental consent, such as a signed check.

Special Requirements for Minors Age 15 and Under
Minors 15 and under must be present at the time of application and need to be accompanied by both parents or guardian(s). If one parent cannot be present, a statement of consent may be notarized and submitted with a photocopy of the front and back of the non-applying parent's ID. If one parent has sole custody, proof of said custody must be submitted (original documents only - ie. death certificates, court orders etc.). Minors 15 and under also must submit both their birth certificate and passport if a passport was previously issued.  Minors may not renew their passport using Form DS-82. Passports for children under age 16 are only valid for 5 years.

Name Changes
If an applicant has had a passport before and has changed their name since the previous passport was issued, proof of the name change needs to be submitted. This can be a marriage certificate or a court order. Original documents only.

Emergency Travel

Please also be advised that we process our last application at 3:30 PM. All applicants and any required parents must be present before this time. All forms need to be filled out before this time and the application must have all required materials present. 


HOW MUCH DOES IT COST?
There will be two payments made - one by check/money order AND one by credit card
  1. The application fee to the Department of State must be made by check or money order AND
  2. The City's processing fee of $35 will be made by Visa or Mastercard
    *Please make check or money order payable to: Department of State and include the applicant's name and date of birth in the memo section. One check per application.
